Accident Reports |

Motor vehicle crash reports are completed
by officers with in 14 days of the report being taken. To obtain a copy
of the crash report, an insurance agent or individual must complete a
"Request For Motor Vehicle Crash Report" form. |

Motor Vehicle Crash Report" form is
available for download by right clicking on the report name. You will need Adobe Reader to
open the form. If you do not have Adobe Reader a free downloadable
version is available at Adobe. |

| All requests for reports must be picked up
in person at the Mansfield Township Police Department between the hours
of 8:30am-4:00pm Mon.-Fri. All request for discovery must be followed up
by a formal letter on law firm letter head to the address below. |
